Dental Access Membership Plan

The Lakeside Dental Access Plan is an in-office dental membership plan for individuals and families who do not have dental insurance. Members pay an annual fee that includes specified preventive or periodontal services and provides savings on many additional eligible treatments performed at Lakeside Dental.

This is NOT dental insurance and CANNOT be combined with dental insurance benefits.

Basic Membership - $360/year | Periodontal Membership - $660/year

Membership is valid for 365 days from the enrollment date and must be paid in full at enrollment.

Basic Membership - $360 per Year

Regular value: $1,165

• Two dental exams per membership year as follows: the initial membership year includes one comprehensive dental exam and one periodic exam; renewal years include two periodic exams
• One full-mouth series of digital X-rays for a new patient or when clinically indicated
• One panoramic X-ray for a new patient or when clinically indicated
• Two professional routine cleanings per membership year, for patients who qualify for prophylaxis
• One fluoride treatment per membership year
• 15% savings on most eligible in-office procedures

Additional qualifying family members may enroll in the Basic Membership for $300 per person, per year.

Periodontal Membership - $660 per Year

Regular value: $1,740. The Periodontal Membership is for patients diagnosed with periodontal disease.

Patients with periodontal disease require the Periodontal Membership in order to participate in the Dental Access Plan.

• Everything included in the Basic Membership, with periodontal care replacing routine preventive cleanings
• 25% savings on scaling and root planing
• One periodontal re-evaluation
• One fine scale cleaning
• Up to four periodontal maintenance cleanings during the membership year
• 15% savings on most eligible in-office procedures

Additional family members needing periodontal treatment may enroll in the Periodontal Membership for $600 per person, per year.

15% Member Savings on Most In-Office Procedures

Members receive 15% savings on most eligible general and restorative procedures performed at Lakeside Dental. The 15% discount does not apply to Invisalign, Vivera retainers, full-mouth implant cases, IV sedation, periodontal surgery, CT/CBCT scans, services provided by outside specialists, or procedures that are already discounted.

** Full-mouth and panoramic X-rays are included no more than once every five years, when clinically indicated and recommended by the treating dentist, while maintaining continuous active membership.

Which Membership Is Right for Me?

Patients who qualify for routine preventive cleanings may participate in the Basic Membership. Patients diagnosed with periodontal disease require the Periodontal Membership. All treatment recommendations, including the type and frequency of examinations, X-rays, cleanings, periodontal treatment, and other services, are based on the clinical judgment of the treating dentist and the patient's individual dental needs.

Dental Access Plan Terms & Important Information

• Membership is NOT dental insurance and CANNOT be combined with dental insurance benefits.
• Membership fees are DUE IN FULL at enrollment, are NON-REFUNDABLE, DO NOT roll over, and membership benefits are NON-TRANSFERABLE.
• Membership is valid for one year (365 days) from the enrollment date. Unused benefits expire at the end of the membership period, have no cash value, and do not roll over to future membership periods.
• Routine cleanings are included only for patients who qualify for a regular prophylaxis. Patients diagnosed with periodontal disease require the Periodontal Membership in order to participate.
• Full-mouth and panoramic X-rays are included no more than once every five years, when clinically indicated and recommended by the treating dentist, with continuous active membership.
• Members receive 15% off most eligible in-office procedures, with the following exclusions: Invisalign, Vivera retainers, full-mouth implant cases, IV sedation, periodontal surgery, CT/CBCT scans, services provided by outside specialists, or procedures that are already discounted.
• Services not specifically included in the selected membership are not included in the membership fee and will be billed separately.
• All treatment recommendations and services are based on the clinical judgment of the treating dentist and the patient's individual dental needs.
• Membership benefits and discounts apply only while membership is active and cannot be applied retroactively to previously completed treatment or outstanding balances.
• Lakeside Dental reserves the right to modify membership fees, benefits, and terms upon renewal.
