Basic Membership

$360 per year

Regular value $1,165. The Individual Savings Plan covers any 1 person, up to 1 person per household.

  • One Full-Mouth Series of Digital X-Rays (new patient/when due)
  • Two Professional Cleanings each year (for patients who qualify for routine prophylaxis)

Periodontal Membership

$660 per year

Regular value $1,740. For patients diagnosed with periodontal (gum) disease. Includes everything in the Basic Membership plus:

  • 25% Savings on Scaling & Root Planing (Deep Cleaning)
  • One Periodontal Re-evaluation
  • One Fine Scale Cleaning
  • Up to 4 Periodontal Maintenance Cleanings during your membership year.
  • Scaling and Root Planing is included once every three years. When clinically indicated, while maintaining continuous active membership.


Program Guidelines

  • Membership is valid for 12 months from the date of enrollment.
  • Membership fees are due in full at enrollment and are non-refundable.
  • This is NOT dental insurance and cannot be combined with dental insurance benefits.
  • Membership benefits are non-transferable.
  • Professional cleanings are provided only for patients who qualify for routine prophylaxis. Patients diagnosed with periodontal disease may upgrade to the Periodontal Membership.
  • Full-Mouth Series and Panoramic X-Rays are included once every five (5) years, when clinically indicated, while maintaining continuous active membership.
  • 15% Discount excludes: Invisalign®, Vivera® Retainers, Full Mouth Implant Cases, IV Sedation, Periodontal Surgery, CT (CBCT) Scans, Already discounted procedures, Services provided by outside specialists
