Please make sure the information you provide is current and complete, particularly your:
- Current medical conditions and relevant health history
- Current medications
- Medication or material allergies
- Dental insurance information, if applicable
- Current contact information
- Emergency contact information
Have Recent Dental Records or X-Rays?
If you have recent dental X-rays or other relevant records from another dental office, you are welcome to have them sent to Lakeside Dental before your appointment. We will review the information available and determine whether any additional records are clinically appropriate.
